Kathy is all nature, no nurture. She was born into this world without the capacity to understand that humans can be good. So she acts cruelly and evilly and self-interestedly because she can't imagine that anyone else is doing anything other than that themselves. The crux of the book becomes clearer a little bit later after her twins surprise their parents.
