You're basically moving the cervix up towards the ceiling, pushing down on the uterus. It's unusual to feel an ovary - it feels like a goldfish swimming under your fingers. But really any tenderness in that exam that's beyond the why I wish you weren't doing this kind of meets your PID criteria.
