The Problem-Oriented Medical Record
The problem-oriented medical record was widely accepted throughout the 70s, to the point that by the early 80s, no one even says problem- oriented medical record anymore. One of the most vocal critics was Alvin Feinstein, who is known as the father of clinical epidemiologists. I would argue that the way that we describe problems is actually driven by some of our bias towards a hyperspecialization. The obsession with filling every single problem can blind us sometimes in the bigger picture.
