A 72-year-old man with gold B COPD presents to the ED. He's been feeling progressively fatigued, lethargic and dissonant. His chest showed up to a large right side of pleurofusion. Also had a large apical nodule with some speculation seen on X-ray. Both were new from prior, and that's why the team was concerned about malignancy.
