Is It Possible to Fix a Bug?

i would like it to be bug free and fast, even if it's in the otpa system. Almost every at that i've worked on, it's usually a handful of bugs that are causing like, 80% or 90%. And so you go fix those things, and yet it gets away better for everybody. We do see sometimes that developers are afraid that things like an ireporting tool is going to be used against them.
