The plan section lays a groundwork for the next steps that are going to come in the client's treatment. The subjective section is basically an opportunity for us to describe our impressions of the client or patient, and it also might be a place that we record the client's perceptions about their care, their progress, or their symptoms. As long as you have information written, that's the most important thing. If you mess up occasionally, it's really fine.
