Tommy, I think the physical exam cannot be stated enough in terms of the importance and understanding what are the features that we can see in ventricular septal defects. VSDs typically cause a holocystolic murmur if the pressure in the right ventricle is lower than the left ventricle throughout systole. Muscular VSDs can have shorter systolic murmurs. Clubbing can actually be quite subtle and you want to check for it as well.
