In a shocked patient, whatever agent you choose, we've ordained ketamine with these miraculous properties of being the ideal drug in every situation. The reality is it isn't, particularly in patients that have a positive shock index. Even the mighty K has those negative ionotropic effects that any drug will have to some degree and we have to be mindful of that. I guess just the one plea that I would make is don't use nothing.
