The first thing you want to confirm, of course, is that this is real. Platelets are one of the kind of notorious lab values that can produce artifact if there's platelet clumping. The way I really approach the kind of question of thrombocytopenia is whether it's a problem with the bone marrow itself and a platelet production problem.
