Is Heparin Exposed to Thrombosis?

Trying to get a history about prior heparin exposure is so key. Even if you don't have that heparin, your body has heparin-like molecules which can still lead to platelet activation and thrombosis. So this cascade can continue on even if you're not getting exogenous heparin. That's why we wait until the platelet count recovers before starting therapeutic anticoagulation.
