Is There Any Guidance on Blood Pressure in Stroke?
If the blood pressure starts systolic start in the 150 to 220 bring it down to about 140 to 150 and you can do that fairly safely within the first hour. If thrombolytic therapy is not going to be used then it's a little more liberal and as long as the blood pressure is less than 220 over 120 then no intervention happens for the first two three days. There are concerns of excessive blood pressure reduction and extension of infarct because of the sensitive the poor auto regulation of the peri-infarct penumbra or the infarCT penumbra.
