How to Achieve the Top Rating

I think if I was in this situation and I was bold and courageous, I would go to my manager and say, thank you for this performance review. You gave me a rating that is lower than I would like to hold myself accountable to achieve each year. I want the top rating and I want to do what it takes to get the top rating. Please help me achieve that this year. And also there was a little more information that we edited out for brevity that the person said it feels like my manager just made something up to put on the form. But it's like make them do their job now.
