How Do You Develop a Novel?

I like to have a couple of twists in my books, and I can't write a twist without having planned towards it. Do you make a deliberate decision that so Nisha's going to be American? Is that for the American market? Does that go on in your head? No, I think there was just a sort of level of grooming. And I always found that women in America had just a different level of care given to their appearance especially later in life. Whereas we Brits tend to be more laid-back, shall we say, about how we put ourselves together.
